We need your help! The
Institute of Public Policy at the University of Missouri is evaluating the
effect of the pilot server training program you are about to complete.
We would greatly appreciate it if you could take a few moments to
complete the survey. By doing so you will help ensure that we have the best information
possible. The survey should take less than 5 minutes to complete. Be assured that your
responses will be kept confidential, and that your participation in this survey is voluntary.
In this evaluation, you will be asked about illegal activities such as underage drinking. The
researchers will keep information about you as confidential as possible, but complete
confidentiality cannot be guaranteed. On very rare occasion, courts have subpoenaed research
records. This research study poses no other foreseeable risks to you as a participant. The
study does provide the benefit of improving the effectiveness of this and other alcohol
responsibility training programs. Your participation in this survey is voluntary. If you
are under 18 years of age, please do not complete this survey.
At the end of this survey, you will be asked to enter your email address for the purpose of
sending you a post-training survey. This is important because in order to determine the effect
SMART has on servers we will need both pre- and post-training survey responses. You will be
entered into a drawing for a $50 gift card upon completion of the post-training survey and you will be
contacted via email by the Wellness Resource Center if you won.
